You need a medication with copper in it. Hole in the head is caused by a protozoan. Coppersafe should take care of it but it'll kill shrimp and snails and some plants.

Also protozoans don't like heat, like above 90 but I'm not sure if your fish can take that. If you increase the heat put in an airstone or two. If that means you need another air pump, get one. Warmer water does not hold as much oxygen so you need to keep lots of bubbles going.

You will have to carry on with the ick treatment till the whitespot has gone, as for the hole have you added any salt as that will help with the whitespot aswell.
