Nothing for years then it all comes at once

The female Ram in our tank has a small patch at the base of her tail on one side of her. It is an off white colour and has a different texture to the surface than her scales - I am not definate that it is a fungus type (although it could be).

Water stats are ammonia and nitrite 0, nitrates under 5ppm, ph7, temp around 83F. High temp due to a. Rams needing a higher temperature and b. a disappointing ick outbreak within the tank.

Regarding the ick, I have been raising the temp gradually over the last few days. Medication has been added, but since both the temp raising and meds added, the ick has left the original fish and I have now noticed new spots on new fish. Some fish have none and one has a few. How have these spores not been killed by the meds when they were free floating? Might I need new/better meds? The medication used had been open for a couple of years. Maybe thats it, a simple shelf life issue.

Should I purchase some Melafix and a (good quality?) whitespot treatment? Any other advice/recommendations?

So update, I went to get some Melafix and new whitespot treatment today. The lfs guy said that it was the brand they used - 'new improved' and 'advanced plus' formula (!) - still Interpet.

It has Formaldehyde in it, do I need to be concerned about this over any particular fish? Will it kill all the snails?

Anyway, will do a 25% water change tonight and bung a net of carbon into the filter - is 12/24 hours enough to clear the water of the old meds before re-treating?

He also said that the Rams patch could be a cyst. Is it best to leave adding the Melafix for this until after the Ick treatment has finished?

A 'cyst' seems a bit vague - had a quick look on Google and can't seem to find anything - anybody shed any further light on this malady? - It looks like a small eroded patch.
