Hmmm, if it's reached this point unfortunately it may not be long for him.

Is this a goldfish? Goldfish sometimes have issues with eating flake food, swallowing air that makes them have problems swimming. This will clear up on its own, but I think some people feed de-shelled peas to their goldfish for digestive problems like this.

There may also be a water quality issue. If your water parameters are out of whack a water change could help, but again if it's reached this point I don't know if there's much that can be done.
