With any carbon insert, once a month. However, you never change the bio-wheel unless it gets unbelievably disgusting, and even then it's best to just rinse it in a bucket of tank water and replace it quickly.

I only change mine...well, I haven't had to yet. I've only had mine maybe a month and a half. Carbon, IMO, is pointless unless you're trying to get rid of a smell, meds, or something else. I rinse it off when I notice the bio-wheel slows down a bit or it looks a little clogged. I have a planted tank so the leaves go up the intake tube once in awhile.

Removing the carbon in favor of filter floss or biomaterial is one thing; leaving it in for unknown periods of time can be dangerous. For one thing, by now it's surely leached back into the water everything it ever picked up.
