Cause is likely to be bacterial , parasitic or dietary, possibly all three. Dont think we can help with this one, but knowing a little more about the conditions the fish lived under might stop it from happening again. Id euthanise asap, that looks damn painful, and he doesnt need to live in that condition of suffering for a moment longer. Bonk him on his little noggin, and dont flush him, make sure he gets wrapped and binned. sorry for the inevitable loss.
