I've had fish in my 20gal hex tank, with a penguin bio wheel, and a tetra airpump rated to 40gal for sometime now. However once in a while one of them dies. Right before they die i notice there belly or were is should be turns real white and they start swimming really funny. Kinda like they are disoreanted, or outa breath. Mostly tetras that do this.

they start losing fins and like becoming deformed. start looking like something froma horror file

These symptoms fit a few different things so I need more info.
What kind of fish and how many are in the tank; is there any cloudiness on their skin; are there any cysts or sores; do the fish flash/scrape their bodies against the gravel or other objects in the tank; have you been feeding them any live food? When you say out of breath do you mean they're lethargic or are they breathing heavily? Any extra information you can provide would be helpful.

Some water quality readings, ammo, nitrite, nitrate,ph etc would be helpful too.

Well I do feed the frozen cubed food once in a while. shrimp,krill,bloodworm. And yes looks like it has sores. No cloudyness , and no scarping. Just swimming kinda sideways like it lost its balance so to speak. It was a black phantom tetra.Im talking lethargic. It also looked like half it mouth had like grown together.
