I do 50%-75% changes weekly or every other week depending on how busy I am. As for removing all the decorations for cleaning...maybe once every 6 months or even once a year. that's up to you really, but you don't want to do a 100% water change unless you're moving or something has contaminated the water that can't be removed with activated carbon. The answer for me is never really, unless I am, as Jason stated, moving a tank. When I changed a community to a fry tank I cleaned it all out, but that was just because I was removing the gravel and it is a HUGE mess. They shade the glow of it with their mossy-misty costumes, They wear masks of silk, porcelain, brass, and silver, So as not to mislead with their own, ordinary faces.
