Ammonia: 0
Nitrite: 0
Nitrate: 40.0
pH: 7.4
Temp: currently 81°; normally 77°
Stock list: rhino plec, 10 giant danios, blue acara, Cuban, snakeskin gourami, 2 clown loaches
Tank Size: 75g
Water change schedule and % changed: once weekly, 20-25%; last change 12/19 ~15%
Filtration: Aqua Clear 110
No new fish, plants, chemicals, etc

I first saw ick in this tank on 12/13. I slowly started raising the temp and treated with Quick Cure, which I've always had success with in the past. After 24 hours I dosed the tank again, according to their directions. No luck.

I've gone thru this treat & water change cycle 3 times now. My Cuban is covered in ick; today I found 2 of my clown loaches dead and the other 2 aren't looking well

I'm really at a loss here as to what I should do next before I lose any more fish

There is a good chance the meds you are using are just out of date, and have lost all the necessary goodies to kill the Ich.

I am not too sure about doing water changes during medication unless recommended by the instructions. (see reasons below) As this can/will weaken the meds strength and could bring more problems. Push the temp up as high as you think it is safe, turn off all lights, reduce feeding, add some form of stress coat providing it is compatible with the meds you are using, and continue the treatment for at least two more weeks.

Did the Ich come through the water (cold weather can make it very active) if so start storing water for your changes otherwise your problem will continue as long as you keep changing the water.

I have asked the Suppliers of Ich Meds in Aust how long can the Ich lay dorment in very cold water and the only answer they could give me was they just dont know but they do know the colder the water the longer it will remain dorment ans as soon as it warms up the out break will happen. That is the only reason I have stored water for over 15+years now. My only Ich problem came via a few Cardinal Tetras I treated it quickly with out any losses.

I did check & fortunately the meds aren't out of date. The bad news is that I lost the plec yesterday and found the remaining two clowns dead this morning. I'm so upset about them

With the remaining fish how high can I safely raise the temp in the tank? I already added an extra airstone.

I'm not sure how long ick can be carried on a fish before it shows (if that makes sense). I bought the plec not too long ago and that's really the only cause I can think of.

I had tetras (head and tailight and rummynose), a BN, yoyos, cories (false juliis), and an otto in my 55g when I had ich a little over a month ago. I raised the temp to 86F and it helped so much. I know some fish are more tolerating than others.
I have ich now and unfortunately since I have pandas now, I can't raise the temp past 82F. I raised it to 84F initially and lost 2 pandas. The only fish affected in my tank now is my German Ram. I got it and my angel a couple weeks ago and I think that is where the ich came from. One of my rummies had it, but it died shortly after a started treatment. I am so tired of treating for ich. I am getting some better meds hopefully today. Good luck. I am sorry about the fish you lost. It makes me so angry and sad to lose fish to something you are trying to fix.
Another trick I am doing is keeping the lights off most of the day. I was told it reduces stress. I started doing it after my 2 pandas died because I didnt want to lose the remaining 3.

edit: I see your stocklist now. I am not sure how high you could raise your temp. I have not been told to raise it any higher than 86F. I think you could slowly raise it to 83 or 84. I would get a second opinion though.
